Referral Instructions

Please complete the form below to refer a client or patient to Sunbird Therapy and Consultation. We accept referrals from NDIS, Medicare, Victims of Crime, and various agencies.

Once the referral is received, we will contact the client/patient via phone or email. If the referral is appropriate, we will arrange an intake appointment.

For NDIS Referrals:
Include NDIS plan details such as the NDIS number, plan dates, and plan manager information.

For GPs:
As an Accredited Mental Health Social Worker, we offer services eligible for a Medicare rebate. Please note that there will be a gap fee, resulting in an out-of-pocket cost for patients.
